At Northern Nephrology, we partner closely with primary care providers to identify and manage chronic kidney disease early, with a focus on slowing progression, optimizing cardiovascular and metabolic health, and preparing patients for the future when needed. We encourage providers to use the form below to calculate your patient’s 5-year Kidney Failure Risk Equation (KFRE) score to help determine whether a nephrology referral may be appropriate. Patients with elevated KFRE risk scores may benefit from earlier nephrology evaluation, multidisciplinary CKD management, and individualized treatment planning.
